Minor discrepancies were found after publication and were fixed in version 1.1. These included some numerical corrections made in the
text of chapters 1, 3, 4, and 5. Corrections in wording were made in chapters 2, 3, and 4. The comparison of 1995 and 2010 results from
chapter 1 was deleted. The appendixes of the original version chapter 3 were deleted, as they were redundant of those in chapter 7.
Coproduct ratios were corrected on the input sheets of chapter 7.
