Minor parts of the text and appendixes were modified. 

Summary of changes made to this document and its contents:
1.	Appendix 1 has been modified: appendix 2 was renamed appendix 1 and the contents of appendix 2 were migrated; table 1-1 was retained. 
2. 	The original appendix 1 contained digital log-ascii standard (*.las) formatted files of borehole geophysical data.  These logs have been migrated to the online database hosted by the USGS Geolog Locator and publicly available at https://doi.org/10.5066/F7X63KT0. The following files were changed during the migration:
	a.	The following logs were adjusted to a consistent depth reference of the top-of-casing: USGS92-13, USGS92-04, LAW-M04, LAW-M03, LAW-M06, PNR-10, PNR-12, PNR-19, PNR-20, PNR-21, PNR-22.
	b.	Incorrect las files for USGS93-3 were provided in former appendix 1.  The correct log for the 1993 collection date has been cataloged on the USGS Geolog Locator; the log collected in 2004 has been retracted for a suspected tool calibration error.
	c.	The file names for USGS92-13 and USGS92-14 (originally described as USGS93-13 and USGS93-14) were corrected.
	d. 	The las files for USGS93-4B and PNR-21 did not contain the full depth record in former appendix 1. The full records have been cataloged on the USGS Geolog Locator. 
	e.	The las header for PNR-10 was corrected for step length.
3.	The contents of original appendix 2, now appendix 1, have been changed as follows:	
	a.	18_USGS93-4Blog93.pdf has been corrected to include well construction and lithologic data; latitude, longitude, elevation, log date, and well depth information were corrected.
	b.	09a_USGS93-3Log93.pdf has been renamed 09-USGS93-3Log93.pdf and corrected to include the correct induction log and to show the correct depth of the geophysical data, to report the 1993 water level, and to repair missing symbology.
	c. 	09b_USGS93-3Log04.pdf has been retracted due to a suspected tool calibration error.
	d.	21_USGS92-13Log04.pdf and 28_USGS92-4Log04.pdf have been corrected to show the correct depth of the geophysical data.
	e. 	12_PNR-20Log04.pdf has been renamed 12_PNR-20Log05.pdf to reflect the correct logging year.
4.	The contents of table 1-1 in appendix 1 have been changed as follows:
	a. 	The table header was revised to reflect current terminology used for National Water Information System (USGS NWIS) station name to be consistent with the search terms available in the USGS Geolog Locator. 
	b. 	Well depth was corrected for the following records: USGS93-4B and MOC-11.
